The Access Center for Independent Living (ACIL) wants you to help with a research study. This study is about the experiences people with disabilities living in Ohio had during the COVID-19 pandemic. People who have clients with disabilities can share their experiences too. Please respond to the survey prompts on your experiences during the COVID-19 pandemic.

This survey will ask you about your background, your disability status, and your experiences during the COVID-19 pandemic. Your experiences will help us better prepare for future emergencies and make healthcare more accessible in Ohio.

This survey is anonymous and voluntary. You can fill it out by yourself or with help from a caretaker or support person. There are two ways to take the survey: clicking the online link listed below, or by asking for a paper copy by emailing [email protected]. If you choose the paper survey, we will mail it to you with a stamped envelope to send it back. Please, do not include your return address on the envelope.
